If your insurance requires a referral, please obtain one from your doctor prior to your visit.

Private Consultation Fees

If you do not have health insurance, or your insurance provider does not cover nutrition services, you are welcome to come in as a private paying patient. Please call for current fee-for-service rates. We offer discounted package rates and installment plans.

We accept cash, checks (made payable to Forest Hills Wellness), and credit cards for your copays and consultation fees.

Policies

A minimum of 24 hours is required for cancellation or rescheduling all appointments. You will be charged a $50 fee for same day cancellations. If you had a family or medical emergency, however, please let us know.
